The haphazard array of narrow categorical programs, cross subsidies and counterincentives had been both extremely unfair and profoundly inefficient. The negative effects on the health and well being of vulnerable uninsured persons, and on the efficiency and viability of health care systems that all Americans rely on, have been well documented by others.
